Fees and Payment Options

Primary services provided, associated fees and payment options are listed below. 

Services and fees are subject to change. You will be provided with a Good Faith Estimate (GFE) after treatment options are discussed with the intake counselor. Services provided may be in person or by telehealth.

Our Space Family Mental Health Counseling PLLC is not paneled with any insurance provider and is private pay only. Our goal as a family counseling center is to keep our rates affordable. The fee structure reflects our desire to provide quality low cost care to everyone so no one is denied needed mental health counseling.

(PLEASE NOTE:  Based on a documented special need, circumstance or status, you may have qualified for a different rate. This fee, as we discussed and agreed to during intake, will be reflected in the good faith estimate (GFE) of the cost of services to be rendered.)

Other Services and Fees. These fees are not eligible for discounts nor are they grandfathered by the start-of-service date. Due to the unpredictable nature of each client’s needs, they will not be included in the GFE. This information serves as notice of additional fees you may incur based on your personal needs and circumstances and specific requests made to the counselors.

  1. Unscheduled/Ad-Hoc/Crisis/Parent Consultation Phone Calls longer than 20 minutes: Charge is prorated based on the agreed upon rate as listed above for adults/teens/elders, children, families or couples, and groups.
  2. Requested Documentation to Include Treatment Summary, and other Written Reports: Charge is prorated at $60/60 minutes.
  3. No Show/No Call to Scheduled Appointment (as defined in the consent form): Charge is based on the agreed upon rate as listed above.
  4. Cancelled Appointment less than 24 Hours (without same-week reschedule as defined in the consent form): Charge is based on the agreed upon rate as listed above.

Health Insurance Waiver.

As discussed during the in-take session and as indicated in the consent form, you understand that Our Space Family Mental Health Counseling PLLC does not accept insurance as a method of payment. By using our services, you understand you are waiving the use of your insurance. You may make arrangements to use your Health Savings Account/Flexible Saving Account for a debit payment. You are responsible for understanding your own insurance benefits as whether or not your insurance carrier will partially or fully reimburse you for out-of-pocket payments made to Our Space Family Mental Health Counseling PLLC. Those amounts may or may not be less than the fees you are agreeing to pay Our Space Family Mental Health Counseling PLLC. 

All counseling fees paid out-of-pocket are made prior to you requesting reimbursement from your insurance carrier.

At any time, you may request a "Professional Receipt" from Our Space Family Mental Health Counseling PLLC. This statement will include Dates of Service, Billing Codes, and Diagnostic Codes. You may submit this statement to your insurance carrier to request partial or full reimbursement of paid out-of-pocket counseling fees. There is no charge for this service.

METHODS OF PAYMENT

In Person: cash, check or money order made out to “Our Space Counseling,” debit card, credit card (MasterCard®, Visa®, Discover®).

By US Mail: check or money order made out to “Our Space Counseling” (do not send cash in the mail).

By Electronic Payment: Zelle®, PayPal®, Venmo®, CashApp®.

  • Payment is due at the end of each in-person or telehealth session.
  • You are responsible for initiating any electronic payment for a telehealth session or a third-party payment on your behalf.
  • Any mailed, emailed or texted request for payment from Our Space Family Mental Health Counseling PLLC is due within 14 days of the issuance date of the request.
  • Your timely payment is appreciated and helps us continue to serve the community at a lower cost.
